Checklist of supporting documents justifying the purpose of a journey to Denmark to be submitted with an application.
Following article 4 in the agreement between Denmark and Ukraine on the facilitation of the issuance of visa to citizens of Ukraine visiting Denmark, Ukrainian applicants will, as a main rule only have to submit the general requirements and the supporting documents mentioned below.
An applicant will have to provide evidence that he/she falls under this category.
For other applicants who can prove earlier Schengen visa (ex original old passport) the general requirements and the supporting documentation mentioned below is as a main rule sufficient. For first-time visitors into the Schengen area, additional documentation may be required, see below.
Please note that submission of documentation do not guarantee issue of a visa but help the visa officer to make a decision.

General requirements:

1 duly completed application form; |
 |
1 recent passport sized photograph, |
 |
Passport with a validity exceeding three months after your scheduled return and previous passport(s) and photocopy of passports and previous visas and stamps (Schengen, Switzerland, UK, USA, Canada,); |

Domestic passport and photocopy of Domestic passport; |
 |
Proof of Travel Medical Insurance covering all Schengen countries with a minimum coverage of 30.000 Euro for: repatriation for medical reasons, urgent medical treatment and emergency admission to hospital treatment during the stay in the territory of the Schengen states (to be provided when applying for the visa), see further information on Medical Insurance.
Please pay attention that for a single entry visa TMI should have 15 more days of validity. |

If foreigner residing in Ukraine, Armenia or Georgia: Valid residence- and/or work permit with a translation into English; |
All supporting and additional documentation should be translated into English before submitting.
Supporting documents:

An original written request from one of the competent authorities for the international road transportation mentioned below*, providing for international road transportation, stating the purpose (incl. to which country the driver is going to), duration and frequency of the trips of the driver conducting international cargo and passenger transportation services to the territories of the EU Member States in vehicles registered in Ukraine (that the vehicle is registered in Ukraine). |
A request should furthermore state the full name, date of birth, citizenship and number of the identity document;

Commitment of the Ukrainian company to cover all costs associated with the trip. |
Additional documentation, only after Embassy request:

Applicants international driving license; |
 |
The Ukrainian, Armenian or Georgian truck company’s license for international truck driving; |

Tax receipt of the Ukrainian, Armenian or Georgian company; |
 |
Proof of professional activity for the invited employed person: Employment letter from the Ukrainian, Armenian or Georgian company stating how long time the applicant has been employed/ date of employment and declaration of salary; |

Work/ Labour book (original and copy); |
 |
Proof of professional activity for the invited self employed person: Certificate of registration of the company in Ukraine, Armenia or Georgia; |
 |
Originals and copies of freight papers, which were stamped by the border control (CMR and PP); |
 |
An original written request from the inviting company, stating the full name, date of birth, citizenship, number of identity document, purpose, duration, date and frequency of the trip of the driver, preferably use the following invitation form.

Step 1
Prepare your application and supporting documents: Go to ‘All about your visas’ link on this website and find out details related to the visa category you want to apply for – here, you can find the applicable fee, documents required, Visa application form, Valid passport (Please note that the passport cannot be older than 10 years and has to be valid for at least three months after your scheduled return) and more.
Step 2
Book an appointment to visit Visa Application Centre: To visit Visa Application Centre you need to Schedule Appointment You can book an appointment online or by calling the Call Centre. Details provided on this website to schedule an appointment.
Step 3
Submit your application: Visit Visa Application Centre as per appointment scheduled and submit your completed Visa Application Forms along with required supporting documents.
Step 4
Pay applicable Fee: Submission Officer will advise you on total payable amount and guide you to the bank cashier where you need to pay in local currency (Hryvnia). Receipt will be provided for all payments made at the Centre.
