Documents Required
General requirements:

1 duly completed application form;
1 recent passport sized photograph,
Passport with a validity exceeding three months after your scheduled return and previous passport(s) and photocopy of passports and previous visas and stamps (Schengen, Switzerland, UK, USA, Canada,);
Domestic passport and photocopy of Domestic passport;
Proof of Travel Medical Insurance covering all Schengen countries with a minimum coverage of 30.000 Euro for: repatriation for medical reasons, urgent medical treatment and emergency admission to hospital treatment during the stay in the territory of the Schengen states (to be provided when applying for the visa), see further information on Medical Insurance.
Please pay attention that for a single entry visa TMI should have 15 more days of validity.
Documentation that travel expenses are covered (Only to be provided when visa has been issued): Return ticket or bus reservation. If travelling by car - copy of green card;
If foreigner residing in Ukraine, Armenia or Georgia: Valid residence- and/or work permit with a translation into English
If you are under 18 years old, you must submit notarized proof of Parents' consent for the travel as well as a translation into English and a birth certificate.

All supporting and additional documentation should be translated into English before submitting.

Supporting documents:

An original written request from the host organization: competent authorities, national sport Federations or National Olympic Committees of the Member States stating the purpose, name, date and duration of the event and name of invited club/team. A list of participants should be enclosed stating full name, sex, date of birth, passport number, function, citizenship and mention of eventual participation fee;
List of participants to be confirmed by the national Ukrainian, Armenian or Georgian sport federation or competent authorities, see Standard list of participants;
Registration form for athletes participating in sports tournaments in Denmark. info about travel route including border crossing in country of final destination, date of flight, flight no, name of airline or bus number plate, (only to be provided when visa has been issued);
Documentation that eventual participation fee has been paid;
Documentation that accommodation is covered;
Documentation that travel expenses is covered;

This provision covers also local and regional public authorities as well.

The list of accompanying persons will be limited to those accompanying the sportsman/woman in a professional capacity: coaches, masseurs, manager, medical staff and head of the sports club.

Supporters shall not be considered as accompanying persons.


Additional documentation, only after Embassy request:
Documentation that the applicant is a member of the relevant sports club/school and for how long time the applicant has been a member, ex membership card or a declaration from the club/school that the applicant is registered;
Documentation that the applicant is a professional sportsman/woman, ex certificate of sport degree from the national federation;
If local sports club, documentation for registration with national federation;
If local sports school, documentation for registration with local authority. If school is part of a sports club, documentation for this;

VISA APPLYING
PROCESS


Step 1
Prepare your application and supporting documents: Go to ‘All about your visas’ link on this website and find out details related to the visa category you want to apply for – here, you can find the applicable fee, documents required, Visa application form, Valid passport (Please note that the passport cannot be older than 10 years and has to be valid for at least three months after your scheduled return) and more.

Step 2
Book an appointment to visit Visa Application Centre: To visit Visa Application Centre you need to Schedule Appointment You can book an appointment online or by calling the Call Centre. Details provided on this website to schedule an appointment.

Step 3
Submit your application: Visit Visa Application Centre as per appointment scheduled and submit your completed Visa Application Forms along with required supporting documents.

Step 4
Pay applicable Fee: Submission Officer will advise you on total payable amount and guide you to the bank cashier where you need to pay in local currency (Hryvnia). Receipt will be provided for all payments made at the Centre.
